Add Tooth Number and Tooth Surface to Claims

There are times when a tooth number and/or tooth surface is required to submit a claim for a procedure. Here is how to enter a tooth number and tooth surface in a Tops Ortho claim.

How to enter tooth number in a claim in Tops Ortho

  1. Go to Tops Ortho > Practice Setup > Fee Schedule.
  2. Place a checkmark in the checkbox labeled Record Tooth Number and Surface with New Transactions.


  1. After you have checked this box, the New Transactions window will offer more fields:
    1. Full fee
    2. Amount charged to insurance. Note: Entering an amount here will increase the expected insurance on the patient's ledger.
    3. Tooth number
    4. Tooth surface

  1. Go to Action > New Insurance Claim.
  2. Select Date Range Claim from the Claim Type dropdown menu.
  3. Select your desired Form from the next dropdown menu.
  4. Click Add New Claims to tops.
  5. You will see the tooth number in box 27 and tooth surface in box 28 of the claim.
