Manage Tooth Number Errors for eClaims

At times a tooth number may be required to submit a claim for a procedure. The related article below explains how to do this. It's important to note, however, that eClaims will be rejected by DentalXChange if the Tooth Number field contains more than two characters. The steps below explain how to manage this rejection.

Tooth number error

The claim below is an example of what will generate when more than one tooth number is entered for a single procedure code. Box 27 lists four tooth numbers.

When you try to e-file such claims, they will reflect a Rejected by Clearinghouse status. The error will state that the tooth number claim item must be less than or equal to 2:

Address the error

  1. Right-click over the claim and select Mark Selected Claim as Voided.
  2. Void the original transaction; see related article Void a Transaction.
  3. Enter the transaction again and enter a single tooth number in the Tooth Number field.
  4. Change the date of the new transaction to reflect the true date; see related article Change a Transaction Date, below.
  5. Generate the claim again by going to Action > New Insurance Claim.
  6. Go to Matrix > Claims and locate the newly generated claim.
  7. Click the checkbox next to the claim and click e-File Checked Claims.

Add remaining tooth numbers in DentalXChange and resubmit the claim

Note: this step can only be completed once the claim has been received by the payer; this typically takes three days.

  1. Log into your practice's DentalXChange account.
  2. Click Claim Search and use Subscriber Search, Patient Search, Claim Search or Advanced Search to find the claim.
  3. Click the blue Claim ID on the left side of the claim.
  4. Once the claim opens, scroll to the bottom and click Edit to Resubmit. A new, revised claim will generate.
  5. Locate the section titled Claim Item Details and click Edit Claim Item Details.
  6. Find the Tooth Number field and click the + icon to add the additional tooth numbers. Click Save when done.
  7. Locate the section titled Claim Information and click Edit Claim Information.
  8. In the Remarks field, add a note such as "Corrected Claim - included additional tooth numbers." Click Save when done.
  9. Click Save and Resubmit.
  10. Return to Tops Ortho and make notes in the patient's chart indicating which claim was corrected and resubmitted directly in DentalXChange.
